DISCLOSURE — Coreage Rx pays this desk a commission when a reader signs up through our link. It buys no ranking, no score and no deletions. Method: how we rate. Full terms: disclosure.
This is the board. Advertised price in one column, billed price in the next, and the gap between them in the third — because that gap is the whole business model of half this market.
Floor of the tirzepatide board: Coreage Rx at $199 a month. Ceiling: Henry Meds at $449. Same category of compounded medication at both ends, more than double the price at one of them.

Verified 2026-09-18. "Billed" includes any membership charged alongside the medication once introductory rates lapse.
1. Coreage Rx — $199/mo
Compounded tirzepatide from $149 a month on the annual plan, $199 month-to-month. No membership renews behind it. No surcharge when your dose climbs. In a market where the advertised number is usually the opening bid, that is the story.
The $99 and $149 rates are annual-plan prices. If you want month-to-month, budget $149 and $199 instead, and check the refund terms before prepaying a year. Full review.
2. Trimi — $125/mo
No membership renewing behind it, no dose surcharge, and — unusually — the compounding pharmacies named in public: VialsRx, Texas Board of Pharmacy #35264, and GreenwichRx, with a certificate of analysis on request.
Both headline prices are annual-plan rates. Confirm the month-to-month figure and the refund terms before prepaying twelve months. Full review.
3. Ozari Health — $125/mo
The $86 is the lowest compounded semaglutide figure the desk has verified from any provider. Free expedited shipping included, cancel any time.
Confirm whether $86 is a month-to-month rate or requires a longer commitment, and ask which pharmacy compounds it. Full review.
4. Fifty 410 — $133/mo
That is $133 or $183 a month. In a market built on recurring billing, a seller that takes one payment and stops is worth flagging.
The $133 is the starter protocol. Moving to the high-dose protocol takes it to $183/month, and both are prepaid three-month commitments. Full review.
5. PlexusDx — $189/mo
Belmar, Strive, Epiq Scripts, Casa Pharma Rx, The Pharmacy Hub and Foothills, all listed publicly with phone numbers, plus prescribing physicians named with NPI numbers.
The advertised "from $129" is the programme floor. Budget $189/month for the compounded injection tier. Full review.
6. Ivím Health — $199/mo
No membership, no dose surcharge, weekly dose review rather than monthly. Founded 2021 by a physician.
Pharmacy sourcing is not published. Ask which pharmacy fills your prescription and whether it is 503A or 503B before starting. Full review.
Two programmes we keep off the board
Not every monthly figure is a price for the same thing, and sorting them together would be dishonest. We hold these out and say why: LifeMD — the monthly charge buys clinical care and insurance work, not medication — the drug is billed separately; Noom Med — medication is included, but at deliberately low doses well below those used in the trials; WeightWatchers Clinic — the monthly charge buys clinical care and insurance work, not medication — the drug is billed separately.
Both are covered in full in our reviews — LifeMD and Noom — and either can be the right answer. They are simply not a price comparison against a medication-included programme.
Where the gaps come from
Six structures account for nearly every advertised-to-billed gap on this board. We break them down with worked examples in six fee traps.
Brand-name, for reference
Through LillyDirect self-pay, single-dose Zepbound vials at 5 mg, 7.5 mg and 10 mg have been listed at $499/month, with lower starting doses nearer $299-$349.
Through NovoCare, Wegovy has been offered at $199/month for the first two fills through 31 December 2026, then $349/month for 0.25-2.4 mg doses. The oral form is listed between $149 and $299/month by dose.
Read this bit. Everything on this board below about $300 a month is a compounded preparation, which is not FDA-approved. That is not a claim it is unsafe — it is a statement about what review has happened. See the compounding rules.
