California eliminated Medicaid coverage of GLP-1s for obesity treatment.

California was one of four states — alongside New Hampshire, Pennsylvania and South Carolina — that eliminated Medi-Cal coverage of GLP-1s for obesity treatment after October 2025, a change KFF attributes to state budget pressure and the cost of the drugs. If you were previously covered in California for weight management, that route has closed. Everything below is sourced — we are not going to tell you what a California clinic charges, because every provider we track is 50-state telehealth and prices the same wherever you live. What actually differs by state is coverage, and that is what this page is about.

Why California is different

California matters disproportionately here because of scale: Medi-Cal is the largest Medicaid programme in the country, so a coverage decision in Sacramento affects more people than the same decision anywhere else. The state also sits in the West, the region with the lowest adult obesity prevalence at 30.2% — which does not make the withdrawal less consequential for the people who were relying on it, but does mean California was covering a smaller share of its adult population than a Southern state would be.

If you are on Medicaid in California

What to do in California: If you were on a covered GLP-1 through Medi-Cal before the change, do not simply stop — talk to your prescriber about whether you qualify under a diabetes, cardiovascular or sleep apnea indication, because those remain mandatory.

Coverage for type 2 diabetes, cardiovascular disease and sleep apnea is mandatory everywhere, so a refusal framed around weight loss is not the end of it — and on Medicare, the $50 programme starts 1 July 2026 regardless of which state you are in.

Paying cash in California

Providers are licensed across all 50 states and charge the same here as anywhere, so there is no local price to hunt for — the cheapest verified all-in rates are Ozari Health at $86 for semaglutide and Trimi at $125 for tirzepatide. If you buy compounded, check the pharmacy licence with the California State Board of Pharmacy, or with the board in whichever state that pharmacy actually operates from — frequently not California.

Did Medi-Cal stop covering Ozempic and Wegovy entirely?

Not entirely. California eliminated coverage of GLP-1s for obesity treatment. Coverage for type 2 diabetes remains mandatory under federal rules, as does coverage for cardiovascular disease and sleep apnea indications. What ended was the weight-management route.

I am mid-treatment on Medi-Cal. What happens now?

Speak to your prescriber before your next refill rather than after it. If you have a qualifying diagnosis the prescription may continue under a different indication; if not, you need a plan rather than an interruption, because restarting at your old dose after a gap is unsafe.
