Wegovy is Novo Nordisk’s semaglutide, approved for weight management. Through NovoCare it has run at $199 a month for the first two fills through the end of 2026 and $349 a month afterwards.

That step from $199 to $349 is what sends most people looking. The alternatives are compounded semaglutide — same molecule, pharmacy-prepared, not FDA-approved — and the Wegovy pill itself, which starts around $149 a month and is approved.

Is compounded semaglutide the same as Wegovy?

Same active ingredient, but it is not Wegovy and it is not a generic. There is no approved generic semaglutide. Compounded versions are prepared by pharmacies and are not FDA-approved.

What is the cheapest route to semaglutide overall?

Compounded programmes start around $86 a month. For Medicare beneficiaries, the $50 monthly programme beginning July 2026 is likely to beat everything.

Caveat from the desk

Compounded preparations are not FDA-approved. Two approved oral GLP-1s now start around $149 a month, and from 1 July 2026 eligible Medicare beneficiaries can access certain GLP-1s for $50 a month — both of which beat a large part of this market. See the approved pills and the Medicare programme.

In STEP-1, adults on 2.4 mg lost an average of about 14.9% of body weight over 68 weeks. That figure comes from a controlled trial of the active ingredient, not from any particular seller's product.
